Silver Peaks offers free and no obligation Senior Placement Services to assist individuals and their family to find the living arrangement that suits them best. We tour, evaluate and partner with many care facilities and senior living communities to help individuals and their family understand their options based on individual care needs, budget, location, and personal preferences.

Types Of Facilities

Independent Living Facilities (ILF)

Assisted Living Facilities (ALF) are designed for individuals who require assistance with Activities of Daily Living (ADL) but don’t require skilled medical services. Residents receive assistance with daily tasks which includes the following but are not limited to: assistance with bathing, dressing, medication management, meal preparation, transportation, incontinence care. Services include daily meals, wellness programs, housekeeping services, laundry services, transportation and social activities and outings. Residents can choos from different rooming options, such as private or shared studio, one or two bedroom apartments. The facility size can vary from 10 rooms to over 200 rooms. This type of facility is generally not allowed to provide skilled nursing services (Administer IVs, maintain catheters, colostomy care, etc) unless there is a credentialed RN or LVN individuals working in the facility.

Skilled Nursing Facilities (SNF)

Skilled Nursing Facilities (SNF) provide services to residents who need skilled services but do not require hospitalization. SNF’s often provide both short term rehabilitation stays for residents recovering from illness, surgery or accidents as well as long term care for residents with more serious medical needs. This type of facility provides 24/7 medical care services, which generally cannot be dispensed in Assisted Living Facilities (ALF) /RCFE’s or Independent Living Facilities (ILF). In addition, SNF’s offers managing complex and potentially serious medical problems such as infections, wound care, IV therapy, and coma care. Services are covered in whole or part, by Medicare or Medicare HMO’s, with the requirement that there is hospitalization for 3 days prior to a discharge to an SNF.

Continuing Care Retirement Community (CCRC)

A Continuing Care Retirement Community (CCRC) also known as a Life Plan Community, is a type of retirement community which combines 3 types of communities (independent living, assisted living and skilled nursing facility) in a single setting. The emphasis of the CCRC model is to enable residents to avoid having to move, except to another level of care within the community, if their needs change. independent living facility, assisted living facility and skilled nursing facility that many CCRCs provide meals, housekeeping, transportation, and health and emergency services as desired by individual residents. CCRC’s require a hefty entrance fee as well as monthly charges.

Board and Care Homes / Residential Care Facilities for the Elderly (RCFE)

Board and care homes also known as Residential Care Facilities For The Elderly
(RCFE) are the ideal solution for individuals who require assistance with activities of
daily living (ADL) such as bathing, dressing, toileting, laundry, medication
management, and extensive personal care assistance. The majority of these types of
homes / facilities hold around 6-8 seniors with 1-3 caregivers on staff at all times.
Residents share communal areas such as a living room and dining room but will
generally have their own private or semi-private room with a handicap accessible
design. This type of facility is not allowed to provide skilled nursing services, such as
give injections or maintain catheters and do colostomy care (unless there is a
credentialed RN or LVN individual working in the home)

Memory Care (Dementia & Alzheimer's Specialty Facilities/Homes)

Memory care is a specialized type of senior living facility that is designed specifically for individuals diagnosed with Alzheimer’s disease, dementia, and other memory loss conditions. Many dementia patients feel the need to wander or have the need to be somewhere. Alarmed doors and emergency call systems are in place for their safety. In addition, memory care facilities incorporate social engagement, memory games, brain fitness activities, specialty diets, and other techniques to help deter memory loss progress while still providing a home like environment. Services may include but are not limited to: medication management, or help with everyday tasks such as dressing, bathing, grooming, and meal preparation.

Hospice Care Services / Facilities

Hospice Care Services / Facilities provides medical services, emotional support, and spiritual resources for people who are in the last stages of a serious illness. Designed for individuals who are facing a terminal condition and/or a life expectancy of six (6) months or less. Services provided include: symptom control and pain management as needed, medical equipment and supplies, counseling and support for the patient and his or her family, guidance in emotional and spiritual issues associated with the oncoming death of a loved one. Hospice care can take place at the individual’s home, hospice care facility, in a hospital, or in a Skilled Nursing Facility (SNF).
